A confirmation of membership letter will be emailed to you as soon as your membership is activated. Download the Momentum App to access your membership records. You do not need a membership card to access your healthcare benefits, your confirmation of membership letter will suffice.
If you need a membership card please send your request to email@example.com. Please note you need to update your personal postal address for the card to be posted to you.
